If you are a prospective new patient, please let us know the general problem you would like us to address, your primary care provider, the location of any relevant imaging, as well as basic insurance information if available.
If you want to get a BIG headstart on the process, navigate to our
PATIENT SPACE page and click one of the intake form buttons to fill out a more detailed history. If you will be joining our pain program, it's the button on the upper left, if you're joining out stellate/ketamine program, the button on the upper right. For the pain program it will be very important to have ready access to the times and dates of other medical visits (PCP/PT, etc) related to your problem. This data is important for insurance authorizations later on, particularly for patients with neck/back pain and commercial insurance.
Some important notes:
We are a non-narcotic practice. If you are seeking a prescriber for opioid pain medications, that is not a service we provide.We are not trained in disability evaluation. If you are filing a disability claim, you are free to submit notes from visits to our clinic, but we do not complete disability forms attesting to your functional capacity or ability to work. If you have insurance that uses copays, deductibles, and/or coinsurance, we require a credit card on file.
Our ketamine and stellate services (for psych and other purposes), as well as regenerative procedures are direct-pay only. We do not bill insurance for these services.Finally, we are located in Portland, Maine -- Not Oregon!
