Regenerative Medicine

Regenerative therapies like platelet-rich plasma and bone marrow-derived stem cells offer hope for ailing tendons, ligaments, and joints.

Ultrasound-guided injections have revolutionized pain diagnosis and treatment. Before ultrasound, the vast array of soft tissue pain generators were challenging to diagnose with accuracy, and hard to treat. Now we can precisely target muscles, tendons, joints, and ligaments in a way we never could before. This makes it possible to accurately diagnose even the tiniest pain generators in the soft tissues. Treatment is now far more effective. What used to take multiple visits can now be accomplished in one. It's truly revolutionary.

Lots of patients come to us unable to tell more about their pain other than the location. "It hurts right...there!" This could be a spot near their elbow, over their shoulder blade, buttock, hip, or knee. Sometimes it's just a seemingly random spot on their foot. These "small" pain problems can arise anywhere. Using carefully guided injection therapy, we can accurately diagnose and treat the tiniest of pain generators. It's not uncommon for patients to suffer for years with problems like these, unable to find relief because conventional medicine just doesn't have the tools. Now we do, and with 10 years of experience diagnosing and treating these small pain generators, Dr. Littlejohn can help finally move your care forward.

Frequently asked questions
Regenerative medicine is a relatively new field that offers therapies designed to restore damaged soft tissues and joints. Regenerative therapies also play a protective role in preventing the progression of degenerative disease like arthritis. Common regenerative therapies include platelet-rich plasma (PRP), which is made from your own blood, and stem cells, which can be derived from a number of different sources.
If you continue to suffer despite having tried all the conventional therapies - rest, ice, heat, massage, exercise, PT, medications, and conventional injectables - that's when it's time to consider regenerative medicine.
The first step is usually a diagnostic injection with anesthetic to demonstrate that your pain is in fact coming from the target site of interest. Pain is complex and may originate where you feel it, or it may be referred from a different source. If you have pain relief with a small volume of local anesthetic, that's good evidence we have chosen the right location for regenerative treatment. The next step is to do an injection of platelet-rich plasma or bone marrow derived stem cells.
The scientific consensus is that it's best to avoid anti-inflammatories for two weeks before the procedure, and at least one week after. Essentially, any medication that has the potential to interfere with platelet aggregation or activation should be held if it is safe to do so.
Patience is key. The natural healing process for the body takes 8-12 weeks, which is what you should expect with regenerative therapies.
For the first two weeks light activity only. New connective tissue is forming and you want to minimize interference in this process. Starting at two weeks, PT with a focus on promoting good healing is optimal. Most physical therapists will know what to do. Expect some discomfort, but adjust your activity level to keep this at a low level. With time and healing, you will tolerate more.
We do a very strong PRP mix that is one of the best available. Expect to pay $1250 for a basic tendon injection, and somewhat more for an extra-high dose, or more sites. We can do alpha-2-macroglobulin as well for a premium. We're not yet offering bone marrow-derived stem cells (BMAC). The cost for this at most sites will run $5-8K per procedure and will be similar here once we offer it.
